Tracheal compression secondary to posterior sternoclavicular dislocation.

چکیده
Posterior sternoclavicular dislocation (PSCD) is uncommon but may cause severe mediastinal injury. We report the case of a patient with PSCD and symptomatic tracheal compression requiring surgical intervention. This was a 16-year-old male with direct chest injury presenting with pain in the right shoulder and cough forty-eight hours after the event. On physical examination, deformity of the sternoclavicular joint and limited abduction (90◦) was observed. The chest X-ray showed posterior sternoclavicular dislocation. Closed reduction was performed and the joint was immobilized with a sling. Fifteen days later, cough, stridor and breathing difficulties persisted. Computed tomography (CT) confirmed right PSCD with severe tracheal compression (Fig. 1).

Figure-of-8 Reconstruction Technique for Chronic Posterior Sternoclavicular Joint Dislocation
Dislocation of the sternoclavicular joint is a rare injury and typically requires high-energy forces applied through the joint. Initial treatment is dependent on the direction of dislocation, with acute reduction indicated for posterior dislocations presenting with signs of tracheal, esophageal, or neurovascular compression. Although most patients do well with conservative treatment after the i...
متن کاملSuperior sternoclavicular dislocation: A case report
INTRODUCTION Sternoclavicular dislocations are difficult to diagnose and often missed. Anterior dislocations are more common than posterior dislocations and typically have a low risk of complications. PRESENTATION OF CASE We report the third case of post-traumatic superior sternoclavicular dislocation, which was successfully treated by functional treatment. DISCUSSION The sternoclavicular j...

Posterior sternoclavicular joint dislocation is a rare but potentially serious injury. It is most commonly seen in teenagers or young adults involved in major trauma and can be difficult to diagnose clinically and radiologically. It is our experience that this injury is frequently associated with a delay in diagnosis and may be clinically insignificant initially. Despite this benign early clini...
متن کاملManagement of traumatic sternoclavicular joint injuries.
Traumatic sternoclavicular joint injuries account for <3% of all traumatic joint injuries. Proper recognition and treatment are vital because these injuries may be life threatening. Injuries are classified according to patient age, severity, and, in the setting of dislocation, the direction of the medial clavicle. Anterior injuries are far more common than posterior injuries. Posterior dislocat...
متن کاملMedial epiphyseal fracture-detachment of the sternoclavicular joint with posterior displacement in a judo athlete – equivalent of posterior sternoclavicular dislocation
Posterior sternoclavicular dislocation is a rare traumatic injury that presents a potential risk of injury to mediastinal structures. The diagnosis is fundamentally clinical and treatment is done on an emergency basis. The authors report the clinical case of a young judo athlete with post-traumatic medial epiphyseal fracture-detachment, with posterior displacement (lesion equivalent to posterio...
